To support the transition to adult care, there are some tools & frameworks that have been developed to help guide families.
One of these is known as the ‘Ready Steady Go ’ transition programme. The programme is aimed at children and young people aged 11 years and over who have a long-term condition and are likely to require ongoing support from adult services throughout their lifetime. It’s programme to help children gain the knowledge and skills to manage their own condition.
The programme is divided into three stages (Ready, Steady, Go), each part of which moves towards helping the young person understand their condition, manage it, and the services available to support them. You can view the programme & documentation on the Ready Steady Go website,here.
