What do we mean by ‘transitions’?

As a child growing up, there may be several occasions where your care may ‘transition’, or move between different organisations. This might be starting at a new school, but often it is referring to moving from children’s care services (e.g. paediatric care) into adult care services.

Preparing disabled children and young people for adult life needs to start as early as possible in order to allow them to develop the skills and knowledge they will need to have choice and control over their lives. There’s a lot of research to show that by providing a supportive and positive transition experience, children are more likely to move into adulthood with good outcomes.

There are a number of barriers that can make this work in practice, and it can require a certain amount of preparation, which should begin several years in advance of turning 18. Coordinating everything can be difficult and you and your family may find different services don’t talk to each other or keep everyone updated. That’s why proactive planning and keeping a clear account of information to share with new care professionals can be helpful.

And talk with other families! Connecting with other parents of slightly older children can be a great source of information, but also emotional support. You’re not alone.
